Chronic disease self-management program returns

The NorWest Community Health Centres has announced that it is relaunching the Chronic Disease Self-Management Program.

According to the centre adults in Thunder Bay and District are experiencing higher rates of chronic diseases such as cancer, diabetes, and cardiovascular than in any other region in Ontario.

The program will utilize evidence-based workshops facilitated by peer leaders, offering resources designed to empower individuals to take an active role in managing chronic diseases.

Workshops offered will focus on the self-management of chronic disease, diabetes, and thriving and surviving with cancer.

“This re-launch marks an exciting chapter for our Chronic Disease Self-Management Program. We’re dedicated to providing individuals with the tools and knowledge necessary to better manage their health conditions effectively,” explained Kevin Ng, manager of the Chronic Disease Self-Management Program. “I am thrilled to lead this initiative and work alongside our dedicated team to make a meaningful difference in the lives of our community members in northwestern Ontario.”
